Admissions Check List


1. Apply for the 2024-2025 School Year

  • Complete the online application to begin the admissions process. 


2. Get in Touch with an Enrollment Advisor

  • Contact an advisor to discuss your student’s needs and learn more about JDLC Leadership Academy.


3. Complete All Required Documents

  • Submit necessary forms such as transcripts, identification, and any additional paperwork requested.


4. Discuss Tuition Options 

  • Review available tuition plans and financial options with your advisor to find the best fit for your family.

Other Required Enrollment Information

Please print and sign all documents below.  If your child does not receive immunizations, you must complete the personal beliefs exemption form.   The affidavit of shared residency is only to be completed if you and your family share a household with another family temporarily.  If you have any questions, please contact us at 480-978-2106
